Choose Your Own Friggin' CSH Adventure was created, scripted, designed, and written by Mark Dezelon. Idea stolen from The Brunching Shuttlecocks (their version is Choose Your Own Damn Adventure).

The script that formats the html/php is written in Python and can be found here. I admit that it's not very well-written. Therefore, I offer no warrenties of any kind. It works for me, and that's all I care about. If you use it, please mention me in the credits.
